I thought the same thing but one of the pictures of her lying down definitely shows her puffing on a cigarette.
When people have a close brush with death, they either change their bad habits or they don't. She's obviously in the second group possibly because of what Scribe said, she's rich and she knows she can cut to the front of the line for another kidney. Who knows.

Not to mention that if the hospital knew she was a smoker, they may have denied her a kidney, or at least made her wait longer as she got passed up by nonsmokers. Like if you are still an alcoholic, you may be denied a liver, as you are just going to destroy the new one, so they might as well give it to someone else. There are only so many available matching organs to go around.
