    I did this one basically by:

    On a fresh layer: Draw in the lines using a path, then fill the path with a predominate color from the image (sky-blue in this case). Filter > Distort > Wave to get some more waveyness - use 1 or 2 generators and keep the other settings low (play with it a bit). Bevel/emboss.

    Select a section from from top to bottom on the original layer that does not include the model (a long rectangle to either the left or right of the model). Copy & paste it to a new layer. Transform the selection (ctr-t) and stretch it across the entire image.

    Magic-wand the layer with the lines to select the area that is not the lines. Go back to the stretched background layer hit delete to get rid of the background where there are no lines. Move this layer to the top. Then Filter > blur > gaussian blur and move the slider until you see the effect you want.

    Other than playing with levels along the way (ctr-l), I think that's it.
